Detailed description
In weight paint mode the levels > offset tool does not add weight to vertices that do not already have some weight.
No blend included as this happens everytime in the build i am using, just add object, go to weight paint mode press levels button in weight paint tools and drag the offset slider to plus one. No change.
Also it might be better to call the operators add and multiply as theres no need for ambiguous terms when these are entirely accurate.

The issue was that on start wpaint mode, no vgroup exists yet. The tools now are greyed out in that case.
It doesn't affect vertices if they have no weight set yes, that's by design and useful.
The name I can't fix now, I agree "levels" is a misleading name.

Sorry, i hadn't considered that there would be no vgroup if following my recreation instruction. My fault. But to put it more clearly, if you were to already have a vgroup, and painted on it in some areas, and the tools are not greyed out, the offset tool works on all the painted areas but has no effect on those with zero weight. This is what i am experiencing.

And that is exactly what the tool is meant to do.
When you make weightgroups for character deformations, you only want to work on the vertices you've already assigned to specific deforms or bones.
