BF-admins agree to remove header information that isn't useful,
to reduce noise.

Developers should add non license comments as separate comment blocks.
No need for separator text.

This is often invalid, outdated or misleading
especially when splitting files.
It's more useful to git-blame to find out who has developed the code.

Turned out there was still quite a few cases were indices were set dirty,
but elem_index_dirty was not tagged accordingly (mostly for BM_LOOP,
but a few others as well). So probably this crash was not the only one
hidden here.
Hopefully all possible cases were catched this time!
bug was introduced in r45297, which inadvertently broke testing for multiple flags at once.
added BM_elem_flag_test_bool() and BMO_elem_flag_test_bool() to get TRUE/FALSE results rather then the flag value.
