BF-admins agree to remove header information that isn't useful,
to reduce noise.

- Keep _size() for size in bytes.
- Keep _count() for data structures that don't store length
(hint this isn't a simple getter).

- `bmesh_radial_faceloop_find_first` & `bmesh_disk_faceedge_find_first`
can be replaced with a single call to a new function:
`bmesh_disk_faceloop_find_first`
- `bmesh_disk_faceedge_find_first` called `bmesh_radial_facevert_check`
which isn't needed, since either the current or next loop in the
cycle is attached to the edge we're looking for.
Separate the creation of trees from EditMesh from the creation of trees from DerivedMesh.
This was meant to simplify the API, but didn't work out so well.
`bvhtree_from_mesh_*` actually is working as `bvhtree_from_derivedmesh_*`.
This is inconsistent with the trees created from EditMesh. Since for create them does not use the DerivedMesh.
In such cases the dm is being used only to cache the tree in the struct DerivedMesh. What is immediately released once
bvhtree is being used in functions that change(tag) the DM cleaning the cache.
- Use a filter function so users of SnapObjectContext can define how edit-mesh elements are handled.
- Remove em_evil.
- bvhtree of EditMesh is now really cached in the snap functions.
- Code becomes organized and easier to maintain.
This is an important patch for future improvements in snapping functions.
add BM_ITER_MESH_MUTABLE which steps before entering the for() loop body and prevents the assert from complaining about removing mesh data while iterating as well as the crash.
this was done in quite a few areas, more may turn up.

added BM_iter_as_arrayN(), returns an iterator as an array without knowing the length before calling.

* BM_mesh_normals_update() now takes skip_hidden as an argument
(previously this was default behavior), however this isnt good when
using BMesh modifiers, where you want all normals to be recalculated.
* add bm_iter_itype_htype_map[], to get the iter type from a BMesh
iterator.
* BM_vert_in_face now loops over a vert's faces rather then every vert in the face.
* rename bmesh_radial_find_first_facevert --> bmesh_radial_find_first_faceloop
* rename bmesh_radial_find_next_facevert --> bmesh_radial_find_next_faceloop
