Short description of error
Cleaning material slots breaks the material index and does not delete unused materials
Exact steps for others to reproduce the error
Using default cube, duplicate 3 more times. Then assign unique material to each one with different colors for visualization. Merge the 4 cubes and go to Material Properties > Right hand side dropdown > Clean Slots > Clean Material Slots This will add new slots and change the order of the existing slots.
