Short description of error
This is likely 1 bug masquerading as 2. The extrusion of the predefined grease pencil line stroke shape does not happen until the mouse is moved. Also, if you click to apply the extrusion, and have not moved the mouse, then the new control point is at the place of the previous control point.
Exact steps for others to reproduce the error
Based on the default startup.

Please note: I tried a screen key tool but it did not work well so I screen key display off.
Note: the mouse did not show up well but you can see the extrude quickly snap to where the mouse actually is (this is part of the problem)
Steps:
Get into workspace: 2D animation.
click on line stroke on left of screen.
left click, drag, left click -> draws a line and works correctly
mouse mouse, stop moving mouse, press E/extrude -> PROBLEM the extruded line does not show up, there should be a line from last point to where the mouse is
move mouse -> now the line segment will appear and snap to the mouse
click and a new/second line segment appears -> correct
second or same problem
move mouse, press E/extrude, click mouse (do not move mouse before click) -> PROBLEM a new endpoint is created that appears to be in the same place as the previous endpoint. The new point should be where the mouse is. You can see this problem at time 26 seconds in the video. The thing to look for is the fact that the red dot/point (point before end) is the same as the yellow dot/point (end). However, the mouse is far to the right of that point (the video failed to capture the tiny mouse in draw mode).

This is consistent with how extrude in Blender works.

If it works as Blender, then it's ok for me, but I would like to know the opinion of @mendio and @pepe-school-land because they use more the tool than I.
Sure, it's okay to follow the same behavior as extrude in Blender
