This happens when transform orientation set to Global
Create a sphere , select a face and extrude
By default it will extrude along the normal , when pressing z it will now extrude along the global but it won't go back to normal when pressing the 'z'key again

When set to Global, Blender toggles between Global and Local when you press X, Y or Z to constrain. This is by design. The issue here is because Extrude via the E-key overrides the orientation set by the user.

@ideasman42 I guess we could make it so it toggles Normal instead instead of Local? In Object mode, Local and Normal are identical anyway.

I think we should simply just use the default transform orientation here. If the user wants to extrude along the normal, they can set it in the transform settings.
Then we don't have any special rules for how the toggle works with certain operators.

the point of the matter is that there is no possibility to switch/alternate back to extrude along normal (in global mode )
