Exact steps for others to reproduce the error
Just take the standard cube and go into Sculpt Mode. Use the settings that are shown in the screenshot. There should be two triangle holes appearing on two sides of the cube.

@ZedDB Here's a test file and a video showing the problem.

I can reproduce with that .blend file but not the default scene.

Do you have any idea what you did to make this happen in your scene?

Really hard for me to know this, since I have used a customised UI since I started using Blender last December. If there is a way for me to reset to standard settings without losing my old stuff, I could try and find out if it's anything I have changed on my end.

You could just be able to load factory default under File -> Defaults -> Load Factory Settings and it should overwrite anything unless you explicitly save your settings after resetting.

I just did what you said and I got the exact same results you got with the factory settings. This is very odd to say the least.

Let me know if you manage to figure out what is causing this as it seems like it must be a scene setting or something. I'm just as confused as you.

@zddb Apparently activating and deactivating symmetry x, y, and z gives completely different results. When all of them and x and y were activated, the cube became whole again. When none were active, only one triangle appeared.

@ZedDB I decided to do a reset back to the factory settings and rebuild my Blender version by adding back my add-ons, favourites, and so on. That seemed to have solved the problem for me regarding the basic cube, but I would wait with closing my bug report for about a week when I have had the chance to work in Blender and QuadriFlow some more. If it appears again despite having done the reset, I will inform you about it.

Yes absolutely! We'll keep this open until you have done some more testing.

Seeing this also. You don't even have to do it in Sculpt Mode.

Edit:
Looks like it's related to the "Use Paint Symmetry" option. If you turn that off, it doesn't make the holes.

cube_remesh_hole.jpg

OK, should be able to follow these steps and reproduce the problem.

1: Select default cube
2: Select QuadriFlow Remesh and change settings to match above.
3: Remesh
4: Cube should be fine
5: Delete cube
6: Add another cube and remesh this one
7: Cube should have holes.
8: Undo
9: Turn off "Use Paint Symmetry" ,cube now has no holes.
10 Undo
11: Turn on "Use Paint Symmetry" ,cube now has holes.

I too have this with my custom setup,Preserve Sharp is the one that gives me two non-manifold tris with the default Cube even with the provided blend file.

@ZedDB The bug reappeared again, sorry to say. I will have to check again what's causing it at some point, but it will take a while probably.

We are aware that the way Quadriflow works has a lot of limitation and it can fail in quite some cases. This is unlikely something that can be fixed for a specific case or model, but something that is going to eventually improve as we continue developing and polishing Quadriflow. I consider the report design limitation of how Quadriflow currently works.
