The empty slot is expected, the jumbling of existing slots and material assignments is the bug as shown in the video

Cycles materials will occasionally turn black in the viewport when deforming meshes

I tried to reproduce it on a system with a RTX 3080 but after many tries the issue still didn't occur - seems like the issue is more likely on weaker systems
